| Part 2 | Additional information Yes or no questions that must all be answered, with an explanation in Part 8 for any yes. Also immigration proceedings, each entry in the past 5 years, and, if abroad, the consulate to notify. |
| Part 3 | Processing information Yes or no questions on criminal, immigration, and security history. A yes may require Form I-192 for a waiver. |
| Part 4 | Spouse and children List family members, including any you plan to include using Supplement A. |
| Part 5 | Petitioner's signature Sign and date. USCIS rejects unsigned forms. |
| Supplement B | Law enforcement certification A certifying official completes and signs it. Submit the original with the I-918 package. Without it, USCIS will deny the petition. |
Common mistakes to avoid
- Filing without an original, signed Supplement B. The instructions say USCIS will deny the petition.
- Leaving out the personal statement. It should describe the crime, when it happened, who was responsible, and the abuse suffered.
- Giving a PO box as the home address. Use the safe mailing address field instead.
- Not filing Form I-192 when a yes answer in Part 3 may make you inadmissible.
- Filing Supplement A for a family member without credible proof of the relationship.
- Not filing Form I-765 with the petition. USCIS recommends it so the bona fide determination can include work authorization.
- Filing Form I-765 for a derivative family member who is outside the United States.
- Not signing the form, or using a typed, stamped, or pasted image signature. USCIS rejects improperly signed forms.
