Form G-28 in short
Form G-28 tells USCIS, CBP, or ICE that an attorney or accredited representative represents you in an immigration matter. Both the client and the representative sign it. It also records where you want USCIS to send original notices and secure documents.
| Issued by | U.S. Citizenship and Immigration Services (USCIS) |
|---|---|
| Current version | Form G-28 09/17/18 (checked 2026-10-25) |
| Who fills it in | An attorney admitted to practice in the United States or an accredited representative of a recognized organization, together with the client. A supervised law student or law graduate signs the same G-28 as the supervising attorney. Attorneys licensed only outside the U.S. use Form G-28I. |
| Who receives it | The DHS agency handling the case: USCIS, CBP, or ICE. |
| Where to send it | File it with the related application, petition, motion, or appeal, online or by mail. A later G-28 for a pending case goes to the office where the case is pending or the return address on the last notice, or online for certain pending forms. |
| Deadline | No set deadline. File it with the related form or when representation starts. A representative standing in at an interview files it in person. |
| Fee | $0 (Form G-1055 Fee Schedule, edition 10/16/26, checked 2026-10-25). |

What changed
USCIS says it will publish a new edition soon. Until then it accepts the 09/17/18 and 05/23/18 editions despite the expiration date printed on the form.
How to fill out Form G-28, line by line
| Part 1 | Information about attorney or accredited representative The representative's online account number, name, mailing address, and contact details. |
| Part 2 | Eligibility information Attorneys give the licensing authority and bar number for every jurisdiction. Accredited representatives give the recognized organization and accreditation date. USCIS rejects a G-28 missing this information. |
| Part 3 | Notice of appearance Select one agency (USCIS, ICE, or CBP), list the form numbers or matter, the receipt number if any, and the client's name, A-Number, contact details, and mailing address. |
| Part 4 | Client's consent and signature The client chooses whether original notices and secure documents go to the representative, then signs in ink. A parent or guardian may sign for a client under 14. |
| Part 5 | Signature of attorney or accredited representative The representative, and any supervised law student or graduate, sign and date in ink. |
| Part 6 | Additional information Extra space, for example for disciplinary disclosures or a U.S. business address for secure documents. |
Common mistakes to avoid
- Missing signatures. USCIS says the most common error is a G-28 that is not signed by both the client and the representative.
- Using a stamped or typed name instead of a handwritten signature.
- Writing the client's name differently from the related immigration form.
- Leaving out bar or accreditation details in Part 2. USCIS rejects the form.
- Selecting more than one agency in Part 3.
- Using highlighter, correction fluid, or graying out parts of the form.
- Using Form G-28 for a matter before the Board of Immigration Appeals. That uses Form EOIR-27.
