Common mistakes to avoid

  • Using one Form 3911 for several refunds. You need a separate form for each refund.
  • Leaving off the spouse's signature for a joint refund. The IRS will not start the trace until both sign.
  • Expecting a replacement after you endorsed the check and someone else cashed it. The form says the law does not allow a replacement in that case.
  • Sending other documents to the Form 3911 fax numbers. The about page says to fax only Form 3911.
  • Writing a name or address on line 4 that differs from the return as filed.
  • Trying to cash a refund check more than one year after it was issued. It can no longer be cashed.
  • Skipping the self-help tools first. The IRS encourages some filers expecting a paper check to start a trace with its online tools.
