Line 1a and 1bName and first SSN, ITIN or EIN
Enter the name shown first on the return. Use the EIN for a business return. For a Form 1040 that includes Schedule C, enter your SSN.
Line 2a and 2bSpouse's name and second SSN or ITIN
Fill in only for a joint return. Either spouse can request a transcript of a joint return.
Line 3Current name and address
Enter your current address, including any P.O. box. If you are incarcerated, include your inmate number.
Line 4Previous address
Enter the address on your last filed return if it is different from line 3. If you have not updated your address with the IRS, the form points you to Form 8822 or 8822-B.
Line 5Customer file number
Optional. Up to 10 digits that will print on the transcript, often used by lenders. Do not use an SSN or name, or the IRS replaces it with 9999999999.
Line 6Transcript requested
Enter one tax form number, such as 1040 or 1120, and check return transcript (6a), account transcript (6b) or record of account (6c). If unsure, the instructions suggest the record of account.
Line 7Verification of nonfiling
Proof that the IRS has no record of a filed return for the year. Current year requests are only available after June 15.
Line 8Form W-2, 1099, 1098 or 5498 transcript

Line 9Year or period requested
Enter the end date of each tax year or period in mm/dd/yyyy format, for example 12/31/2024 for a calendar year 2024 return. List each quarter for quarterly returns.
SignatureSignature, attestation box, date, phone and title
Check the attestation box, then sign and date. Sign exactly as your name appeared on the return. Only one spouse needs to sign for a joint return. Entities enter a title and attach proof of authority.

Common mistakes to avoid

  • Leaving the attestation box in the signature area unchecked. The form says it will not be processed if the box is unchecked.
  • Signing before all lines are complete. The form says not to sign until every applicable line is filled in.
  • Mailing the form more than 120 days after signing it, which leads to rejection.
  • Entering more than one tax form number on line 6. Use one form number per request.
  • Putting an SSN or name in the customer file number on line 5.
  • Expecting the transcript to go to a lender or other third party. The IRS mails transcripts only to your address of record.
  • Using Form 4506-T when you need an actual copy of the return or a W-2 attached to it. That needs Form 4506.
  • Sending it to the wrong RAIVS address. Use the chart that matches the transcript type and where you lived when the return was filed.
