How to fill out Form W-4P, line by line

Step 1(a) and (b)Name, address and SSN
Your name, address and social security number. Without an SSN the payer withholds as if you were single with no adjustments.
Step 1(c)Filing status
Single or married filing separately, married filing jointly or qualifying surviving spouse, or head of household. This sets the standard deduction and tax rates used.
Step 2Income from a job or other pensions
Use the online estimator, or enter pay from jobs in 2(b)(i) and lower paying pensions or annuities in 2(b)(ii). The form gives four worked examples on page 2.
Step 3Dependents and other credits
If total income will be $200,000 or less ($400,000 if married filing jointly), multiply qualifying children under 17 by $2,200 and other dependents by $500, then add other credits.
Step 4(a)Other income
Income not from jobs or pensions, such as interest or dividends, that you want tax withheld on.
Step 4(b)Deductions
The result from the Deductions Worksheet on page 4 if you expect deductions beyond the basic standard deduction.
Step 4(c)Extra withholding
Any additional tax you want withheld from each payment.
No withholding boxChoose no withholding
Check it to have no federal income tax withheld, then complete only Steps 1(a), 1(b) and 5. U.S. citizens and resident aliens generally cannot choose this for payments delivered outside the United States and its territories.
Step 5Signature
The form is not valid unless you sign it.

Common mistakes to avoid

  • Using Form W-4P for a lump sum or rollover eligible distribution. Use Form W-4R for those.
  • Completing Steps 3 to 4(b) on every pension form. Fill them in only on the form for the highest paying pension, or on Form W-4 for your job if you have one.
  • Leaving out a spouse's job or pension when married filing jointly.
  • Forgetting to sign Step 5, which makes the form invalid.
  • Checking the no withholding box for payments delivered outside the U.S. as a U.S. citizen or resident alien.
  • Not updating other pension forms after starting a new pension, when the form's tip says to submit new ones.
