Common mistakes to avoid
- Not sending VA Form 21P-0969 when you have over $75,000 in assets, transferred assets in the last three calendar years, or have 5 or more income sources. Section IX says to submit it.
- Reporting income twice. If you complete the 21P-0969, the form says to report only Social Security income on this form.
- Leaving income items partly blank. The form says skipped items are treated as unspecified income and may delay the claim.
- Claiming in-home or facility care costs without the care worksheets on pages 16 and 17 completed by each provider.
- Claiming Special Monthly Pension without VA Form 21-2680 from a physician, PA, CNP or CNS, or VA Form 21-0779 for nursing home residents.
- Sending original documents. The form says VA cannot return originals.
- Signing with an X without two witnesses.
