VA Form 20-0995 in short
VA Form 20-0995 asks VA to look again at a decision you disagree with, based on new and relevant evidence. | Who fills it in | A veteran, or a non-veteran claimant such as a surviving spouse, child, parent or fiduciary, who received a VA decision and disagrees with it. VA says you must have had a prior decision on the claim and the claim cannot be a contested claim. An accredited representative can sign if a VA Form 21-22 or 21-22a is on record. |
| Who receives it | The VA intake center for the benefit type you check in item 1, such as the Compensation Intake Center or the Pension Intake Center. |
| Where to send it | Online: only for disability compensation claims, through the Supplemental Claim form on VA.gov. VA says you can also bring non-healthcare claims to a VA regional office. |
| Deadline | The form instructions say you have one year from the date of the decision notice letter to file a Supplemental Claim and keep eligibility for the earliest effective date for any benefits granted. If you are responding to a Statement of the Case or Supplemental Statement of the Case in the legacy appeals system, the form says you must submit it within 60 days of that document to move into the modernized review system. |
| Fee | No VA filing fee. The form notes that an accredited attorney or claims agent can generally charge a fee only after VA has issued a decision on the claim. |

How to fill out VA Form 20-0995, line by line
| Item 1 | Benefit type Check only one benefit type, such as Compensation, Pension/DIC/Survivors, Education or Veterans Health Administration. If you disagree with decisions on more than one benefit type, file a separate form for each. |
| Section I, items 2 to 10 | Veteran's identifying information The veteran's name, Social Security number, VA file number, date of birth, service number, insurance policy number if any, address, phone and email. VA says it can still identify you if you give at least the last name and SSN or VA file number. |
| Section II, items 11 to 19 | Claimant's information Fill in only if the person filing is not the veteran, for example a surviving spouse, child, parent or fiduciary. Include relationship to the veteran. |
| Section III, items 20A to 20D | Homeless information Complete only if you are homeless or at risk of becoming homeless. VA uses this to prioritize your request. Add a point of contact VA can reach. |
| Section IV, items 21A and 21B | Issues for Supplemental Claim and decision dates List each issue from your decision letter that you want reviewed, with the date of the VA decision notice for each. Only the issues you list will be reviewed. |
| Section V, items 22A to 22D | New and relevant evidence Attach new evidence or tell VA where you were treated so it can get records. VA can get VA and military treatment records without a consent form. For private providers or Vet Centers, also send VA Forms 21-4142 and 21-4142a. |
| Section VI, item 23 | 5103 notice acknowledgment Applies to compensation, pension, DIC and accrued claims filed more than one year after the decision. Read the evidence page on VA.gov and check Yes, or check No and VA will mail you the notice. |
| Section VII, item 24 | MST notification option Only for compensation claims tied to military sexual trauma when you are enrolled in VA health care. A response is optional. |
| Sections VIII to XI, items 25 to 29 | Signatures The veteran or claimant signs item 25A and dates it. If signing with an X, two witnesses sign in Section IX. An alternate signer or a power of attorney signs only in the cases the form describes, with the required VA forms on record. |
Common mistakes to avoid
- Checking more than one benefit type in item 1. The form allows one benefit type per form.
- Filing without any new and relevant evidence and without naming records for VA to get. Unless the claim is based on a change in law, VA says the application is not complete without it.
- Sending evidence VA already had. New evidence means information VA has not considered before.
- Leaving out issues. Only the issues listed in item 21 are reviewed.
- Not sending VA Forms 21-4142 and 21-4142a when you want VA to get private medical records.
- Mailing to the wrong intake center for the benefit type.
- Using this form when your condition has worsened. The form says to file VA Form 21-526EZ for an increase instead.
- Waiting more than one year after the decision letter, which can cost you the earliest effective date.
