How to fill out VA Form 10182, line by line
| Part I, items 1 to 9 | Personal information Veteran's name, VA file number and date of birth. If you are not the veteran, add your own name and date of birth. Then your mailing address, phone, email and representative's name. Check the box if you are experiencing homelessness. |
| Part II, item 10 | Board review option Check only one: 10A Direct Review with no new evidence and no hearing, 10B Evidence Submission within 90 days, or 10C Hearing with a Veterans Law Judge. For a hearing, also choose central office, videoconference or virtual telehearing. |
| Part III, item 11 | Issues to appeal List each issue from your decision notice and the date of that decision. Say what you disagree with, for example service connection, the rating, or the effective date. Only issues you list will be considered. |
| Part III, checkboxes | Early decision and extension request Check the first box if you want the Board to decide as soon as possible, which means you cannot change review option later. Check the second box if you are asking for more time to file for good cause, and attach your reasons. |
| Part III, item 11C | Additional issues and sheets Use this space for more issues, and check the box if you attach extra sheets. Put the veteran's last name and file number on each sheet. |
| Part IV, items 12 and 13 | Signature and date The appellant or an appointed representative signs in ink and dates the form. |
Common mistakes to avoid
- Missing the one-year deadline, or the 60-day deadline for a contested claim. The form warns that filing late could mean losing the right to appeal.
- Checking more than one review option in Part II. You must check only one. To use different options for different issues, file a separate form for each option.
- Leaving out an issue. Only the issues listed in Part III are considered.
- Not giving the date of each decision you are appealing.
- Choosing Direct Review and then trying to send new evidence. Direct Review does not allow new evidence.
- Using this form to add new evidence for a VA rater or to ask for a senior reviewer. The form says to use VA Form 20-0995 or 20-0996 for those.
- Forgetting the veteran's last name and file number on attached sheets.
