Common mistakes to avoid
- Printing double-sided. State does not accept double-sided forms. Print on two separate single-sided pages.
- Forgetting to sign and date page 1.
- Not sending the most recent passport. You must mail it with the form. It is returned separately.
- Using DS-82 for a child under 16 or for a passport issued when you were under 16. Those need Form DS-11 in person.
- Using DS-82 when the passport was issued more than 15 years ago or is lost, stolen, or damaged. That needs DS-11.
- Not stapling the photo. State says to staple it with four staples vertically in the corners, close to the photo edges.
- Paying a $35 facility fee or letting a post office review the form. State says DS-82 is mailed and postal employees should not charge that fee.
- Sending cash. Use a personal check or money order with the applicant's full name and date of birth on the front.
Questions about Form DS-82
Can I renew my passport online instead?
Yes, if you qualify. You must be 25 or older, renew a 10 year passport that expires within 1 year or expired less than 5 years ago, not change your name or sex, be in a U.S. state or territory, and not need to travel for at least 6 weeks. Online renewals cannot be expedited. The only official site is opr.travel.state.gov.
Who can renew by mail with DS-82?
People who can send their most recent passport, were 16 or older when it was issued, got it within the last 15 years, have an undamaged passport not reported lost or stolen, and have the same name or a document showing the name change.
Do I sign DS-82 before mailing it?
Yes. Sign and date it after you print it. This is different from DS-11, which you sign in front of an agent.
Can I print DS-82 on both sides?
No. State says to print on single-sided paper and does not accept double-sided forms.
Will I get my old passport back?
Yes. State returns the most recent passport in a separate mailing, which may take up to 2 weeks after the new passport arrives.
Can I renew a child's passport with DS-82?
No. Passports for children under 16 cannot be renewed. Apply with Form DS-11 in person.
