Common mistakes to avoid
- Filing it for an expired passport. The form says an expired passport is no longer valid and you do not need the form.
- Mailing it to CLASP while also applying for a new passport. In that case submit it with the DS-11 instead.
- Leaving out the front and back photocopy of your photo ID when mailing it.
- Trying to travel on the passport after reporting it. It is cancelled and you may be detained on entry.
- Keeping a reported passport you later find. The form says to return it for cancellation and not use it.
- For a child under 16, having only one parent sign without proof of sole authority.
- Mixing up the passport book and card numbers, which are different.
