Italian Modello 69 (Deed Registration Request) in short
Modello 69 is the request form generally used in Italy to register written deeds at an Agenzia delle Entrate office. For property leases and rentals the RLI form is used instead. For loans for use (comodato), preliminary sale contracts and profit distribution minutes, the RAP form is also available.

What changed
Since 1 January 2025 registration tax is self-assessed: the deed is registered on the date of the request, after the tax worked out by the parties has been paid.
How to fill out Italian Modello 69 (Deed Registration Request), line by line
| Header | Office, repertory number, date of signing The office where you file, the notary repertory number if the deed was recorded by a notary or official, and the date the deed was signed. |
| Quadro A | General data: type of deed and action Give the type of deed, for example sale, exchange, comodato, and tick Reg for a registration. Pro, Ces and Ris are for extensions, transfers and terminations of leases. |
| Quadro A | Person requesting registration Tax code of the person asking for registration. |
| Quadro B | Parties affected by the deed For each party a running number, tax code, full name or company name, place and date of birth, sex and tax address. For married women give the maiden name. |

Common mistakes to avoid
- Using Modello 69 to register a property lease or rental, for which the Agenzia points to the RLI form.
- Not paying registration tax before the request: since 2025 the tax is self-assessed and registration happens after payment.
- Filling in Quadro C, which is for the notary, official or office.
- Leaving out a party's tax code in Quadro B or abbreviating names.
- Forgetting to sign one of the completed sheets.
- Not numbering the sheets when a section needs more lines.
- Filling in Quadro D for a deed that does not involve leases, rentals or comodato of real estate.
Questions about Italian Modello 69 (Deed Registration Request)
Do I need Modello 69 to register a free loan for use (comodato) in Italy?
Modello 69 is the general form for registering written deeds. For comodato contracts the Agenzia delle Entrate also points to the RAP form. Quadro D of Modello 69 is for comodato of real estate.
Can I use Modello 69 for a rental contract?
For property leases and rentals and later changes, the Agenzia points to the RLI form.
Where do I hand in Modello 69?
At an Agenzia delle Entrate office. Non authenticated private deeds can be registered at any Agenzia office.
When is registration tax paid?
Since 1 January 2025 it is self-assessed: you pay first and the deed is registered on the date of the request. If the office finds more tax is due, it sends a payment notice.
How many pages does the form have?
3 pages. The sheet with the mandate and Quadri E and F is only for leases of residential units with deed code 7202.
