Form I-693 in short
Form I-693 reports the results of an immigration medical examination to USCIS. It shows that an applicant is not inadmissible to the United States on health-related grounds. Most people applying for a Green Card with Form I-485 must submit it, completed and signed by a USCIS-designated civil surgeon.
| Issued by | U.S. Citizenship and Immigration Services (USCIS) |
|---|---|
| Current version | Edition 01/20/25 (checked 2026-10-25) |
| Who fills it in | The applicant fills out Part 1 and signs Part 2 in front of the civil surgeon. A USCIS-designated civil surgeon completes and signs the medical parts. The applicant, not the civil surgeon, submits the form to USCIS. A separate form is needed for each applicant. |
| Who receives it | USCIS. |
| Where to send it | Submit it with Form I-485. If you file Form I-485 by mail, include Form I-693 in the original sealed envelope from the civil surgeon and send it to the Form I-485 filing address. If you file Form I-485 online, open the sealed envelope, upload the completed Form I-693, and keep the original and the envelope until USCIS decides your case. Other applicants follow the instructions of the office that asked for the exam. |
| Deadline | Since Dec. 2, 2024, USCIS says Form I-693 (or a partial Form I-693, such as the Vaccination Record) must be submitted with Form I-485, or USCIS may reject the Form I-485. |
| Fee | USCIS filing fee $0 (Form G-1055 Fee Schedule, edition 10/16/26, checked 2026-10-25). The civil surgeon sets the fee for the medical exam, and fees vary. The applicant pays all exam costs, including any follow-up tests or treatment. |

What changed
The edition is 01/20/25. USCIS accepts only the 01/20/25 edition if the civil surgeon signs the form on July 3, 2025 or later. The USCIS Policy Manual says a Form I-693 signed on or after Nov. 1, 2023 is generally valid only for the benefit application it is submitted with.
How to fill out Form I-693, line by line
| Top of each page | Name and A-Number Write your name and A-Number (if any) at the top of every page. The civil surgeon checks that it matches Part 1. |
| Part 1 | Information about you Your full legal name, physical address, date and place of birth, sex, A-Number and USCIS Online Account Number (if any). Item 4 is a box for refugees, derivative asylees and K visa holders who only need the vaccination record part. If you have no A-Number, type or print N/A. |
| Part 2 | Applicant's statement, contact information, certification, and signature Do not sign until the civil surgeon tells you to. You must sign in the presence of the civil surgeon. A parent or legal guardian may sign for a child under 14. |
| Parts 3 and 4 | Interpreter and preparer Completed only if someone interpreted the form for you or prepared it for you. That person signs their own part. |

| Parts 6 to 10 | Civil surgeon sections and Vaccination Record The civil surgeon summarizes the exam, signs a certification, completes the worksheet and the Vaccination Record (Part 10). A doctor or clinic you are referred to completes Part 9. |
| Part 11 | Additional information Extra space for any answer that does not fit. Add your name, A-Number, and the page, part and item number. |
Common mistakes to avoid
- Signing Part 2 before the appointment instead of in front of the civil surgeon.
- Using a doctor who is not a USCIS-designated civil surgeon. USCIS will not accept the form.
- Accepting or mailing the form without a sealed envelope, or opening the envelope when filing by mail. USCIS returns the form if the envelope is opened or altered.
- Filing Form I-485 without Form I-693 or a required partial Form I-693. USCIS may reject the Form I-485.
- Using an old edition. Forms signed by the civil surgeon on or after July 3, 2025 must be the 01/20/25 edition.
- Leaving the name and A-Number off the top of each page.
- Not bringing vaccination records and photo ID to the first appointment.
Questions about Form I-693
Can any doctor do the exam?
No. Only a doctor designated by USCIS as a civil surgeon can do it, with limited exceptions for some health departments and military physicians. You can search for one with the USCIS Find a Doctor tool.
Which parts do I fill out myself?
Only Part 1 and the name and A-Number at the top of each page. You sign Part 2 in front of the civil surgeon. The civil surgeon and any referral doctors complete the rest.
How much does the exam cost?
USCIS charges no filing fee for Form I-693. The civil surgeon sets the exam fee, and you pay it and any follow-up costs directly.
I filed Form I-485 online. What do I do with the sealed envelope?
Open it and upload the completed Form I-693 with your online application. Keep the original form and envelope until USCIS decides your case, because USCIS may ask to see them.
I had a medical exam abroad as a K visa holder or refugee. Do I need a new one?
Often not. Certain refugees, asylee derivatives and K-1 to K-4 visa holders may only need a partial Form I-693 (Parts 1 to 5, Part 7 and the Part 10 Vaccination Record). The I-693 instructions list the conditions.
How long is Form I-693 valid?
The USCIS Policy Manual says a form signed by a civil surgeon on or after Nov. 1, 2023 is generally valid only for the benefit application it is submitted with. Check the Policy Manual for your case.
