LP1F in short
Form LP1F is the paper form used to make and register a lasting power of attorney (LPA) for property and financial affairs in England and Wales. It lets you (the donor) choose people you trust (attorneys) to make decisions about your money and property, such as running bank accounts, paying bills or selling your home. The same form is used to apply to the Office of the Public Guardian to register the LPA, and it cannot be used until it is registered.
| Issued by | Office of the Public Guardian (OPG) |
|---|---|
| Current version | LP1F Property and financial affairs (08.25) (checked 2026-10-25) |
| Who fills it in | The donor fills in sections 1 to 7 and signs section 9. The donor must be 18 or over and have mental capacity when making the LPA. Either the donor or the attorney(s) can then fill in sections 12 to 15 and apply to register it. The donor does not need to live in the UK or be a British citizen. |
| Who receives it | The Office of the Public Guardian (OPG), which checks and registers the LPA. After registration, the attorneys use it with banks, building societies and other organisations. |
| Where to send it | Post the original signed LPA with the fee (or fee reduction form) to: Office of the Public Guardian, PO Box 16185, Birmingham B2 2WH. If you use the online service, you still print the LPA, sign it on paper and post it to OPG. |
| Deadline | There is no fixed deadline to register. The form says it is a good idea to register straight away, because mistakes cannot be fixed if the donor loses mental capacity first. GOV.UK says registration takes 8 to 10 weeks if there are no mistakes, and this includes a statutory waiting period of 4 weeks. |
| Fee | GOV.UK lists the fee to register an LPA as £92, or £184 if you register both a property and financial affairs LPA and a health and welfare LPA. A repeat application within 3 months after OPG could not register the LPA costs £46. You may pay half if the donor's income before tax is under £12,000 a year, or nothing if the donor gets certain means-tested benefits (apply with form LPA120). Fees are non-refundable. |

What changed
The registration fee rose from £82 to £92 on 17 November 2025. The current form (08.25) no longer prints the fee and tells you to check GOV.UK or call 0300 456 0300 for the current amount. The fee reduction form LPA120 was updated in February and April 2026. The Powers of Attorney Act 2023 allows OPG to introduce a modernised LPA service, including identity checks, but OPG's 2025 to 2026 annual report says this is still being designed and tested, and no go-live date had been published when this page was checked.
How to fill out LP1F, line by line
| Section 1 | The donor Your name, any other names you are known by, date of birth, address and optional email. You are the donor: the person giving the power. Write in capital letters with a black pen. |
| Section 2 | The attorneys Name, date of birth and address of each attorney. You need at least one. Attorneys must be 18 or over, have mental capacity, and must not be bankrupt or subject to a debt relief order. Use Continuation sheet 1 for more than 4. A trust corporation can be an attorney on this type of LPA. |
| Section 3 | How should your attorneys make decisions? Only if you have more than one attorney. Tick one: jointly and severally (they can act together or alone), jointly (they must agree every decision), or jointly for some decisions and jointly and severally for others (list the joint decisions on Continuation sheet 2). |
| Section 4 | Replacement attorneys Optional. People who step in if an original attorney can no longer act, for example because they die, lose capacity, become bankrupt or are no longer your spouse or civil partner. The same age and bankruptcy restrictions apply. |
| Section 5 | When can your attorneys make decisions? Mark one option: as soon as the LPA is registered (and also when you lack capacity), or only when you do not have mental capacity. The form notes that the second option can make the LPA less useful, because attorneys may be asked to prove you lack capacity each time. |
| Section 6 | People to notify Optional. Up to 5 people who are told when the LPA is being registered, so they can raise concerns. You cannot name your attorneys or replacement attorneys here. |
| Section 7 | Preferences and instructions Optional. Preferences are wishes attorneys should keep in mind. Instructions are rules they must follow. The form warns that instructions that are not legally correct would have to be removed before registration. Most people leave this blank. |
| Section 8 | Your legal rights and responsibilities Everyone signing the LPA must read this page. It explains the Mental Capacity Act 2005 principles that attorneys must follow. |
| Section 9 | Signature: donor The donor signs and dates this page in front of a witness, before anyone else signs. The witness must be 18 or over and must not be an attorney or replacement attorney. Sign any continuation sheets 1 and 2 at the same time. |
| Section 10 | Signature: certificate provider Signed after the donor. The certificate provider confirms the donor understands the LPA and is not being pressured. They must be 18 or over and either have known the donor personally for at least 2 years or have relevant professional skills, such as a GP or solicitor. |
| Section 11 | Signature: attorney or replacement Each attorney and replacement attorney signs a separate copy of this page, after the certificate provider has signed section 10. Each signature must be witnessed by someone 18 or over who is not the donor. Attorneys can witness each other. |
| Section 12 | The applicant Who is applying to register: the donor, or the attorney(s), not both together. If attorneys appointed jointly are applying, they all need to sign section 15. |
| Section 13 | Who do you want to receive the LPA? Choose who OPG sends the registered LPA and letters to: the donor, an attorney, or someone else. You can also choose how they prefer to be contacted, including in Welsh. |
| Section 14 | Application fee Choose to pay by card (OPG will phone you, so do not write card details on the form) or by cheque. Tick the boxes here if you are applying for a reduced fee with form LPA120, or making a repeat application within 3 months. |
| Section 15 | Signature (application to register) The applicant from section 12 signs and dates this last, after sections 9, 10 and 11 have been signed. By signing, you confirm you have sent form LP3 to any people to notify. |
Common mistakes to avoid
- Signing in the wrong order. The donor signs section 9 first, then the certificate provider signs section 10, then the attorneys sign section 11. The guide says OPG will not register the LPA if the order is wrong.
- Using an attorney or replacement attorney as the witness for the donor's signature. The donor also cannot witness an attorney's signature.
- Choosing a certificate provider who is not allowed, such as a family member, in-law, unmarried partner, business partner or employee of the donor or an attorney.
- Signing a photocopy or using a digital signature. GOV.UK says everyone must sign the same original document.
- Leaving out pages, including blank pages. The checklist says to send every page of the LPA.
- Using correction fluid. The form says to cross out mistakes, rewrite nearby, and have everyone involved initial each change.
- Choosing 'jointly' for all decisions without appointing a replacement. The form warns that if one joint attorney can no longer act, the LPA stops working.
- Forgetting to send form LP3 to the people to notify before applying to register.
Questions about LP1F
Can I make a property and financial affairs LPA online?
Yes. GOV.UK has an online service where you fill in the LPA and pay. You then print the LPA, everyone signs the paper copy in the right order, and you post it to OPG. GOV.UK says it is usually quicker if you make it and pay online.
How much does it cost to register an LP1F?
GOV.UK lists the fee as £92 for one LPA, or £184 for both types. You may pay half if the donor earns under £12,000 a year, or nothing if they get certain means-tested benefits. Check GOV.UK for the current fee before you pay.
Who can be my certificate provider?
Someone aged 18 or over who has known you personally for at least 2 years, or someone with relevant professional skills such as your GP or a solicitor. They cannot be an attorney, a relative, a partner, or a business partner or employee of you or your attorneys.
Who can witness the signatures?
Anyone aged 18 or over, with limits. Your attorneys cannot witness your signature, and you cannot witness theirs. Attorneys and replacement attorneys can witness each other. A certificate provider can also act as a witness.
Can my attorneys use the LPA before it is registered?
No. The LPA cannot be used until OPG has registered it. GOV.UK says this takes 8 to 10 weeks if there are no mistakes.
Is an LP1F valid in Scotland or Northern Ireland?
No. GOV.UK says an LPA is legally binding only in England and Wales. Scotland and Northern Ireland have their own arrangements.
